- Step #1: Download and complete the transcript request form (CLICK THE BANNER ABOVE)
- Step #2: Submit form to Clovis High School Registrar located in the front office. Please have your ID.
- Step #3: Submit form and copy of ID in-person, via email or fax (Registrar contact information listed below)
